Woman dies in crash on Toronto-bound QEW in Burlington

A woman has died after a three-vehicle crash on the Toronto-bound QEW in Burlington.
It happened around 4:30 p.m. Monday at Burloak Drive.
The OPP said the injured were all in one vehicle, a GMC SUV, which had five occupants.
Rassina Al-Yafi, 65, of Calgary, was thrown from the SUV and died at the scene.
Another passenger, believed to be a 70-year-old man from Calgary, was airlifted to Hamilton General Hospital with chest injuries.
A 33-year-old man from Mississauga suffered minor injuries while a 32-year-old woman from Mississauga suffered serious injuries. A 13-month-old baby boy escaped unharmed.
The drivers of the other two vehicles — a man, 29, from Mississauga and a man, 28, from Niagara-on-the-Lake — were not injured.
A cause of the accident hasn’t been determined.
The highway was closed for several hours at Appleby Line as police investigated.
All lanes have since reopened.
