Proxy appointment forms now available for October election

TORONTO, Ont. – The City of Toronto has released proxy appointment forms for Toronto’s October 25 election.
If an elector cannot vote during the voting period, for any reason, they can appoint someone as a proxy to vote for them. The proxy must be someone who is an eligible and trustworthy elector in the city.
An elector can serve for another qualified elector, or one or more qualified electors who are relatives (i.e., parent, grandparent, child, grandchild, brother, sister, spouse or the elector appointed), using separate proxy forms for each.
The City will mail proxy appointment forms to electors, which can be requested by phoning 416-338-1111.
Proxy appointment forms are also available from City Clerk’s Office locations, Monday to Friday from 8:30 a.m. to 4:30 p.m.

-Toronto City Hall, 100 Queen St. W., 1st floor north
-Etobicoke Civic Centre, 399 the West Mall, main floor
-North York Civic Centre, 5100 Yonge St., ground floor
-Scarborough Civic Centre, 150 Borough Dr., 3rd floor
-Toronto Elections Office, 89 Northline Rd.
The elector appointed must present both copies of the proxy appointment form in person along with acceptable identification of both the elector making the appointment (photocopies acceptable) and the elector appointed (original copy) to the City Clerk or designate for certification.
Proxy appointment forms must contain original signatures and must be certified by the City Clerk’s office to be accepted at the voting place. Election officials will not accept faxed or photocopied proxy forms.
